Please get ready for your visit.

Bring the following to your first appointment:

  • We accept most major insurance plans. Please bring your current medical insurance identification card 
  • A list of current medications 
  • Any radiology films or CDs and any biopsy slides. Your physician will not send these; you must bring them with you to your appointment. 
  • Your driver's license or some form of identification 
  • Co-pay: please refer to your insurance company for proper payments. We accept cash, credit/debit cards, and checks. 
  • New patient packet (For new patients or for patients who have not been seen for more than 3 years): This is mailed to new patients by our medical secretaries prior to their first appointment with us. Web version coming soon!

If you are seeing a specific provider for the first time , please print the specific packet and have it filled out before your appointment.
